The Community Health Worker (CHW) serves as a vital link between Sanitas and the surrounding community. Collaborate with local organizations to create and promote community activities. Facilitate self-sustained groups, identifying community leaders. Connect residents to resources through community connectors. Support high-need patients by linking them to appropriate resources. Collaborate with the medical center team and participate in weekly meetings. Report to the Community Coordinator in Florida.Skills & Qualifications: High school diploma or equivalent; certification as a Community Health Worker in Florida required. 1 year of experience in community development and/or outreach. Knowledge of local community needs and effective information gathering. Strong public speaking and administrative skills. Proficiency in Microsoft Office Suite required.
