PRIMARY FUNCTION : The Community Health Worker (CHW) will oversee and support the JeffMD Clinical Experience students as they learn to speak with and advocate for their patients. The CHW will coach and guide these first- and second-year medical students through their experience working with patients on social needs related to their health. The CHW will also lead discussions with students during case-based learning sessions pertaining to social needs. When the students are not in clinical sessions, the CHW will continue to interface with patients and their families in order to facilitate access to community resources and services. The CHW will provide social support, informal counseling, education on the various community resources and services, and advocate for individual and community health needs. The CHW will be a liaison between the patient/families and the community service agencies. The CHW will coordinate with service providers and health plans, as appropriate, to obtain care for the patient. The CHW will provide support to the selected practice care coordinators and providers through a team-based care model. ESSENTIAL FUNCTIONS: Facilitates first- and second-year medical students as they participate in social determinants screening sessions at various clinical sites; provides guidance and supervision to medical students as they assist patients with identified social needs. Leads case-based learning sessions with medical students. Assists in developing and implementing care plans in cooperation with patients, families, social workers, case managers, and care providers, and serves as a liaison between patients/families/communities and healthcare agencies. Identifies and helps to enroll eligible individuals in federal, state, and local private or nonprofit health and human services programs. Provides referral and follow-up services. Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson. OTHER FUNCTIONS AND COMPETENCIES : Enhances patients and families ability to effectively communicate with healthcare providers and understanding to carry out healthcare providers' advice. Follows-up whether patients are receiving and adjusting well to their care plans, providing additional resources as necessary. Provides culturally and linguistically appropriate health education materials within the scope of training. Documents all patient outreach activities in Epic (electronic medical record database) and a HIPAA-compliant case management system. Participates in continuing education as required by certification(s). Track student progression across platforms including Microsoft Excel and Canvas. QualificationsEDUCATIONAL/TRAINING REQUIREMENTS : High School Diploma or GED CERTIFICATES, LICENSES, AND REGISTRATION :Certified Community Health Worker training program preferred.EXPERIENCE REQUIREMENTS :Prior experience in community outreach preferred.ADDITIONAL INFORMATION :Proficient in entire Microsoft Office Suite, learning management systems (i.e. Canvas, Blackboard), online meeting platforms (i.e. Zoom, Microsoft Teams), and cloud content management (i.e. Box, Dropbox, Microsoft OneDrive).Conditions of EmploymentCovid Vaccination is a requirement for employment at Jefferson for employees working at Jefferson's clinical entities or at the University.
