Job Title: Community Outreach Specialist CalAIM Program
Location: Fresno, CA
Position Overview:
We are seeking a motivated and experienced Community Outreach Specialist to support the CalAIM program. The ideal candidate will have a background in outreach, community health, and patient enrollment, with a strong ability to engage and educate the community. This position will play a crucial role in enrolling individuals in the CalAIM program and increasing awareness within the community.
Key Responsibilities:
- Conduct outreach activities to enroll eligible individuals in the CalAIM program.
- Educate the community about program benefits and services.
- Collaborate with local clinics, hospitals, and community organizations to establish outreach opportunities.
- Set up and manage outreach tables at various locations.
- Drive to different community sites for outreach efforts, maintaining a flexible schedule.
- Track and report on enrollment metrics and outreach activities.
- Work independently while utilizing provided resources and training to effectively engage with the community.
Qualifications:
- Outreach experience, particularly within the CalAIM program or similar health initiatives.
- Strong interpersonal skills with a go-getter mindset and ability to work independently.
- Valid driver's license and willingness to travel to various locations within the community.
- Bachelors degree in Social Work, Psychology, Sociology, or a related field (preferred) with at least 2 years of relevant experience; equivalent experience may be considered in lieu of a degree.
- Community Health Worker certification is highly desirable.
- Familiarity with local community resources and the ability to leverage existing networks for outreach.
Compensation:
- Hourly pay: $25 - $30 per hour, commensurate with experience.
- Monthly bonus potential: $2,500 for every 50 patients enrolled.
- Mileage reimbursement for travel to outreach locations.
Work Schedule:
- Monday to Friday, 6 hours per day (8 AM 3 PM or 9 AM 4 PM with a 1-hour lunch).
Additional Information:
All necessary materials (tables, chairs, marketing materials) will be provided to facilitate outreach activities. Successful candidates will receive training on the workflow and best practices for engaging with potential enrollees.
