LPR International is seeking a Community Health Worker (CHW) to be a frontline advocate for health and wellness supporting our client throughout PG County, MD. This is a long-term temporary assignment, with an immediate start, through July 31, 2025.What You'll Do:As a Community Health Worker, you will be the bridge between residents and the resources they need to live healthier lives. Your work will directly impact the well-being of individuals and families by:
  • Assisting residents with enrolling in crucial health and social service programs.
  • Supporting residents in setting and achieving personal health goals.
  • Encouraging healthy behaviors and lifestyles within the community.
  • Referring residents to medical transportation and other essential services.
  • Offering social support and connecting clients to professional counseling when needed.
  • Guiding residents through health education, including managing chronic health conditions and disease prevention.
  • Coordinating care and case management, ensuring seamless access to health services and follow-up care.
