The Community Health Worker will be a member of a multidisciplinary clinical care team involving inpatient and outpatient providers and social workers. They will engage persons from vulnerable populations who are not established in the healthcare system to transition between and within various healthcare settings, serving as liaison between the medical teams to build a therapeutic relationship, coordinating care with health and social services providers in partnership with Dell Medical School, including but not limited to UTHA, Seton/Ascension, CommUnityCare, Lone Star Circle of Care, and community partners, and connecting patients with unmet social and medical resources.Mediation Among Individuals, Communities, and Health and Social Service Systems. Providing Appropriate Health Education and Information, Care Coordination, Case Management, and System Navigation. Providing Coaching and Social Support. Advocating for Individuals and Communities. Building Individual and Community Capacity. Providing Direct Service. Implementing Individual and Community Assessments . Conducting Outreach, Participating in Evaluation and Research. Promote policy and systems changes.PurposeWork with multidisciplinary team and other local health system and community partners to help patients achieve good health practices, and promote community outreach.ResponsibilitiesService Coordination, Navigation and Direct Service Skills:* Connect patients to primary care medical homes, including assistance with follow-up appointments and care coordination between inpatient and outpatient teams, both medical and social work/case management. Work directly with CATCHH (CHWs Addressing Transitions of Care for Hospitalized unhoused patients) program, which focuses on hospitalized houseless patients nearing, and post-, discharge in effort to improve transitions of care.Capacity Building and Advocacy Skills:* Engage patients and family in discharge planning, health promotion, and health education. Support and provide general health related education and counseling, especially in unhoused patients. Promote individual and collective empowerment through health education, skill development, advocacy, organizing and collaboration with strategic partners.Education and Facilitation Skills:* Use learner-centered teaching to provide one-on-one education on wellness and disease management and encourage patients to adopt self-management skills. Schedule and/or deliver health education classes in designated communities.Individual and Community Assessment Skills; Outreach, Evaluation and Research Skills:* Conduct outreach to houseless individuals, who have recently been discharged from an inpatient setting, with high level of medical complexity. Provide assessment and documentation of patients' health-related social needs and assets; participate in family and community assessments. Coordinate with team members across various project requirements. Assist with developing assessment and evaluation tools for project longevity and maximum positive patient outcomes. Will also be responsible for documentation of patient needs, resources connected to, encounters, and follow-ups using software tools adapted with the University and care teams. As this documentation will be used for program evaluation and assessment, accuracy and timeliness will be critical.Communication and Organizing Skills:* Coordinate with community partners, serve as liaison between Dell Medical School, Dell Seton Medical Center, UTHA, CommUnityCare, Lone Star Circle of Care, and other community partners, utilize written and verbal communication, and assist with building a foundation for long term solutions. Work closely with above partners to improve the local health care system's ability to serve houseless patients during transitions of care.* Other related duties as assigned.Required QualificationsHigh school/GED and 5 years of community/health experience OR Bachelor's degree and 1 year of community/health experience. Experience working with individuals from historically marginalized populations, particularly unhoused patients. Demonstration of interviewing techniques to obtain personal information, to make inquiries and referrals, and to resolve conflicting information. Working knowledge of community-based resources including health and social services, eligibility criteria, and how to access them. Relevant education and experience may be substituted as appropriate.Preferred QualificationsDemonstrated high-level interpersonal and relationship building skills. Excellent verbal and written communication skills, including verifiable documentation skills. Excellent community engagement skills, including ability to engage in courageous conversations and advocate for policy and systems change to improve community conditions. Strong patient-centered mindset in working with internal and external partners. Given the position's focus on working with patients who are unhoused, an applicant's lived experience being houseless will be considered highly valuable. Experience working for or with local health care and/or community-based organizations. Certification by the TX Department of State Health Services as a Promotora/Community Health Worker. Ability to work independently and as a part of a team in implementing project goals. Ability to use computer equipment and tablets and to learn new computer software/programs. Ability to drive with a valid Texas Driver's License and access to a reliable car to travel within the local Austin area. Knowledge of HIPAA/OSHA/ADA/FERPA compliance. Knowledge of resources that serve individuals who are older, with a disability and/or other barriers including MAP, SSI, Medicaid and Medicare. Understanding the non-medical drivers of health including, but not limited to, economic, structural, historical, or other barriers to adequate care. Adherence to highest ethical, legal, and regulatory standards for community health workers.
