The Community Health Worker is responsible for helping community residents and their families to navigate and access community services, other resources, and adopt healthy behaviors. This position supports individuals, families, and communities in developing and meeting health enhancement goals. Community Health Workers are expected to progress in their learning and training by obtaining future certifications issued by the State of California and funding partners. Position Details: Community Health WorkerLocation:Woodland, CA Hours:Full Time- May include work during days, evenings, nights, weekends and holidaysCompensation: $22.00 - $26.00 per hour, depending on experience Essential Duties:
  • Supports individuals, families, and communities in developing and meeting health enhancement goals
  • Provides CHW services through individual or group sessions.
  • Performs outreach to the county population in an assigned service area to provide education on health services.
  • Provides care coordination services by assisting clients in identifying service needs and appropriate resources.
  • Makes appropriate in-house or out-of-agency referrals.
  • Assists eligible clients in securing available financial assistance for healthcare-related expenses.
  • Provides health education, coaching, support, and services focused on general health maintenance and promotion.
  • Supports department with networking activity by proactively partnering in community activities or events.
  • Assists clients in getting the most out of their Primary Care Physician (PCP) and specialty provider visits by conducting pre and post-visit contacts.
  • Provides accompaniment to PCP, specialty, and social service visits as needed to provide emotional support and gain further understanding of the client's deficits when engaging with service providers.
  • Provides screening and assessment services that do not require a license.
  • Assists clients in preventing the onset or exacerbation of a health condition.
  • Evaluates client's levels of health literacy and translates communications with providers and nursing staff.
  • Conducts assessments including, but not limited to CHA, PHQ9, and BAM.
  • Evaluates client's needs and referrals to appropriate resources to address social determinants of health on an ongoing basis.
  • Provides medication review with clients.
  • Provides assistance in applying for and connecting with public and community benefits and resources, including educating patients on qualifying criteria for various benefits and resources.
  • Travels throughout the County.
  • Performs other duties as assigned or otherwise identified.
Requirements: Education/ Work Experience:
  • Possession of a high school diploma or equivalent is required.
  • Certification as a CHW or at least two (2) years of experience as a CHW, Promotor, or Health Navigator; or two (2) years' experience working in community outreach, customer service with a medical office, Behavioral Health, or Substance Use disorder program.
  • Must have lived experience that aligns with and provides a connection between the CHW and the Member or population being served. This may include, but is not limited to, experience related to incarceration, disability, homelessness, mental health conditions, or substance use disorders, or being a survivor of domestic or intimate partner violence or abuse and exploitation. Lived experience may also include shared race, ethnicity, sexual orientation, gender identity, language, or cultural background with one or more linguistic, cultural, or other groups in the community for which the CHW is providing services.
  • Willing to pursue further education, certification, and requirements issued by the State and/or funding partners.
  • Must possess current and valid California driver's license, including a driving record acceptable to F&H insurance company.
  • Must pass criminal background screening.
  • Completion of a pre-employment drug screening and post-employment drug or alcohol tests upon reasonable suspicion of use.
  • Must be insurable by F&H insurance carriers.
  • Provide required proof of legal right to work in the United States.
  • Must provide T.B. clearance from a medical professional.
  • Current CPR/First Aid Certification
  • Ability to multi-task, take initiative, work independently, and solve problems.
  • Excellent interpersonal and customer service skills.
  • Excellent verbal and written communication skills; detail-oriented and organized.
  • Proficient in Microsoft Office Suite or related software.
  • Possess reasoning and problem-solving skills, with emphasis on seeking solutions.
  • Strong organizational skills and detail-oriented.
  • Ability to effectively handle stress and pressure consistent with the job duties and industry.
