DescriptionPOSITION SUMMARY The Community Health Worker (CHW) plays a critical role in promoting health and wellness within the community by providing outreach, education, and support services to individuals and families. - Conduct outreach activities to identify and engage individuals and families in need of health services.
- Build and maintain relationships with community members, healthcare providers, and local organizations.
- Participate in community events, health fairs, and other outreach activities to promote health education and services.
- Provide culturally appropriate health education to individuals and groups on various health topics, including disease prevention, nutrition, mental health, and chronic disease management.
- Develop and distribute educational materials and resources to the community.
- Advocate for clients' needs within the healthcare system and connect them with necessary social services and resources.
- Empower clients to take an active role in managing their health and well-being.
- Collect and record documentation of services provided, and outcomes achieved.
- Collect and report data on community health needs and program effectiveness.
- Work collaboratively with healthcare providers, social workers, and other team members to coordinate care and services for clients.
- Participate in team meetings, training sessions, and professional development opportunities.
- All other duties as assigned.
SKILLS & ABILITIES Education: High school diploma required. Bachelor's degree or higher in health education, social work, or a related field preferred.Experience: Minimum of 2 years of experience as a Community Health Worker or similar role.Computer Skills: Able to learn and utilize electronic data systems and electronic communications. Proficient in Microsoft Office.Certificates & Licenses: Certified Community Health Worker (CHW) preferred. Valid driver's license and reliable transportation.Other Requirements: Strong interpersonal and communication skills, with the ability to build trust and rapport with diverse populations. Knowledge of local healthcare and social service systems. Ability to work independently and as a part of a team. Excellent organizational and time management skills.#IND3
