DAY SHIFT 8:00AM-4:30PMJOB SUMMARY: Provides direct services by means of education, advocacy and services coordination to individuals in order to extend psychosocial support services.ESSENTIAL FUNCTIONS:
- Assessment and Planning Gathers information related to individual and family, financial, health, and emotional status in order to develop plan for addressing psychosocial barriers to health and well-being of clients to develop a client directed Service Plan outlining service.
- Education/Support Provides education and psychosocial support on issues such as health, medical compliance, budgeting, safety, parenting and community resources according to assessed need and resulting plan.
- Advocacy/Services Coord. Maintains information on available community resources, makes appropriate and timely referrals for services, and provides ongoing coordination of needed services.
- Documentation Maintains all necessary documentation required by hospital, department and funder.
- Outreach In collaboration with other staff, provides information and referral to community at large as well as individual patients both in the community and in the home.
REQUIREMENTS: Formal working knowledge; equivalent to an Associate's degree (2 years college); requires knowledge of a specialized field.
