To be a leading force for change in the health, economic and social status of our community.SUMMARY:Community Health Worker (CHW) are part of the Pediatric Behavioral Health integration team. The CHW will work closely with the BHI clinician and Pediatric team to help families connect to needed supports to optimize their children's social emotional development with emphasis on cultural competence CHW will not diagnose or formally treat mental health problems. CHW will help families recognize ways to improve their children health and social /emotional development. The CHW should be comfortable with working with schools, Early intervention and other community agencies as well as the administrative and clinical staff who care for children at DHH GENERAL DUTIES & RESPONSIBILITIES:Responsible for working with families:
- Educates families about mental health screening and caring for children with emotional, behavioral and developmental needs
- Supports caregivers develop skills needed to advocate for their child, navigate the healthcare system, and coordinate their child's care
- Helps families understand and communicate about their child's mental health concerns in a culturally appropriate way.
- Helps families interact with community service agencies and access community services and resources
- Works with community-based organizations on behalf of clinic families, helps families' complete applications for community services.
- Conducts home visits with families and site visits to local community agencies.
- Help families overcome barriers to care for routine health care, chronic medical concerns and mental health problems.
Performs a variety of follow-up duties such as:
- Be available for onsite consultations, with and without BHI
- Maintains notes and records in the EMR
- Maintains positive relationships with collaborating and consulting health care providers and community agencies.
- Assists team in making referrals and supporting families with follow-up assessments
CHW tasks: a. Outreach
- Initiate and maintain trusting relationships with children and families
- Conduct home visits and site visits to community partners
b. Individual and Community Assessment
- Conduct preliminary screening and assessments of child and family
- Assess for barriers to accessing services
- Help children and families identify their goals, personal strengths
c. Assist in the development and implementation of care plansd. Provide emotional support and education to children and families to support wellness and foster resiliencyEducation to Promote Healthy Behavioral Changee. Provide education and support to promote healthy behavior changef. Promote efforts to prevent injury and disease through risk reductionCare Coordination and System Navigationg. Support children and families to effectively navigate the health care environment and access community resources and servicesh. Assist the child, family, and care team in coordinating services through transitions in care, referrals to specialty services and follow up visitsi. - Encourage children and families to develop organizational and leadership skills needed to advocate for care and services
- Build and maintain community networks and participate in activities to build further capacity for services within the community
RequirementsREQUIRED SKILLS & COMPETENCIES:Education: High school diploma or GED, some post- high school/GED training preferred.Experience: At least 1 year of experience directly related to the duties and responsibilities specified. Bicultural with language capacity in Spanish or Vietnamese preferred.Certification/Licensure: Valid Driver's License, Verifiable good driving record and reliable transportation.Software/Hardware: Basic computer knowledge. Experience with an electronic medical records system.
