The Community Health Worker (CHW) supports the care management and coordination activities of the care team in the CIN. What you will do. Mail educational materials and letters, schedule patient appointments, locate and obtain outside patient records. Collaborate with community partners, such as VNA agencies, caregiver programs, and DME providers. Use general knowledge of medical conditions, nutrition, and diet to educate patients. Assist in locating and updating patient clinical records. Facilitate patient and caregiver access to community resources programs, Meals on Wheels, senior centers, chore services, etc. Screen patients to assess for depression, substance use, and mental health disorders; respond with appropriate interventions and/or referrals. Minimum Qualifications. High school diploma. Medical Assistant Certificate preferred. Associates degree or equivalent medical experience preferred.
