JOB SUMMARYThis position is responsible for assisting patients and other individuals by assessing the needs and resources available within our communities; providing social support and informal guidance regarding navigation and access to community resources; conducting home and environmental assessments to evaluate and address potential barriers to care in support of access to care at our health centers and care plan implementation; participates as an active member of the care team; functions as a support resource for patients in addressing social determinants of health and assists with follow up of patient goals; provides Outreach and Enrollment services assisting patients and families to obtain, maintain and sustain enrollment in their insurance plan of choice.ESSENTIAL JOB DUTIESReasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.* Participates as a functional member of the multidisciplinary care team within the primary care and behavioral health departments. Specializes in patient and family engagement in the process of assessing individual strengths and needs. Utilizes knowledge and skills associated with cultural competency to achieve trust and build supportive relationships with patients. May assist in the identification at risk patients and works closely with behavioral health specialists and other members of the care team.(15%)* Assists with initial client/patient intake and assessment to identify healthcare and social service needs utilizing Social Determinants of Health guidelines. Is an active participant in care team meetings, huddles and other communication to discuss patient care and exchange relevant information in the process of defining and reviewing care plans and other patient goals. (20%)* Responsible for the ongoing communication with patient/clients in the form of face to face encounters, home visits, phone and/or electronic means etc. to support the patient/client accessibility to care which may include barriers to care. The CHW is a key resource to the care team in communicating updated information on current outcomes or achievement related to outlined care plan goals or other agreed upon objectives for healthy outcomes. (15%)* Provides guidance and support for a patient/client needing assistance in the health plan application process. This may include information and support in directing patients/clients to the Social Security Administration, Department of Health and Human Services, and educate and support needs for securing transportation. CHW's provide ongoing information on how to successfully navigate the GLBHC community health center environment but also community resources and services to enable and enhance individual success. (15%)* Provides supportive health counseling, education and information, social support, and advocacy for patients and families. Assists patients/clients in building their individual capacity to address health and social issues. Supports the patients' needs through education coordination along with the care team to meet the patients' medical needs. (10%)* Responsible for the comprehensive, accurate, timely and appropriate documentation including electronic data entry within all applicable systems including but not limited to: the electronic medical record, data management systems and any others as required by contract or requirements. (10%)* Responsible for outreach as defined by our third party payer incentive and value based program requirements and agreements. Contacts assigned patients needing services based upon payer reports and other GLBHC analytic reports. Responsible for the tracking and follow up related to patient appointments, emergency department use and missed or cancelled patient appointments. Duties, responsibilities and activities may change at any time with or without notice.MARGINAL JOB DUTIES* Responsible for the planning and participation in community outreach activities, health fairs and other GLBHC supported events.* Assists in the orientation and training of newly hired Community Health Workers.* Provides Community Health Worker support for migrant and seasonal farmworker activities as needed. Promoting services at events where the use of the mobile medical and dental units are used.* Performs other duties as assigned.REQUIRED JOB SPECIFICATIONSEducation: High School Diploma or GED required. Associate degree preferred.Licensure: Basic Life Support (BLS) certification Required.Experience: One (1) year of work experience in health services, public health or other similar area required, in excess of three to five (3-5) years preferred.Skills: Excellent verbal and written communication skills; problem solving skills; ability to utilize cultural competency skills to engage persons of diverse backgrounds with cultural compassion, sensitivity and competence; ability to use computer applications to include electronic medical records, Microsoft word, Excel, and database entry; understanding of medical terminology. Flexibility to accommodate evenings and weekends when necessary.Interpersonal Skills: Able to communicate effectively with, and relate to diverse populations in a professional and courteous manner. Demonstrated customer service skills, teamwork and problem solving, independence in job organization and be able to demonstrate initiative.Physical Effort: Must be able to lift, carry, push, pull, and or twist while holding up to 25 lbs. occasionally.Hours of Work: Full-time/Part-time; flexible and varied. Must maintain regular and timely attendance. Some evenings and weekends may be required.Travel: Reliable transportation, valid driver's license and liability auto insurance is required. May require travel within service area and between GLBHC site locations.
