Job Description:
The Community Health Worker (CHW) serves as a bridge between the community and healthcare providers to promote health, reduce disparities, and improve access to healthcare services. The CHW works closely with individuals and families to provide culturally appropriate health education, support, and advocacy, enhancing community health and wellness.Essential FunctionsReasonable accommodations may be made to enable individuals with disabilities to perform these essential functions.Outreach and Education:* Conduct outreach activities to identify individuals and families in need of health services.* Provide health education on various topics, including chronic disease management, nutrition, mental health, and preventive care.* Organize and facilitate community health events, workshops, and support groups.Advocacy and Support:* Advocate for individuals and families within the healthcare system.* Assist clients in navigating healthcare and social service systems.* Provide emotional support and build trusting relationships with clients.* MNsure Navigator CertifiedHealth Promotion and Disease Prevention:* Implement and support community-based health promotion and disease prevention programs.* Distribute educational materials and resources to community members.* Conduct home visits to assess health needs and provide health education and resources.Case Management:* Assess client needs and develop individualized care plans.* Coordinate and follow up on healthcare appointments, referrals, and services.* Monitor and document client progress and outcomes.Data Collection and Reporting:* Collect and maintain accurate client data and records.* Prepare and submit reports on activities, outcomes, and community health trends.* Participate in program evaluation and quality improvement efforts.Key Competencies* Experience working with diverse populations and underserved communities.* Strong interpersonal and communication skills.* Ability to build trust and rapport with clients from diverse backgrounds.* Knowledge of community resources and social services.* Ability to work independently and as part of a team.* Proficient in using electronic health records and other computer applications.* Bilingual skills (English/Spanish, English/Hmong, or other languages) are highly desirable.Work EnvironmentPrimary environment is home office, administrative office, or clinical office. This role requires regular walking.
