Employment Type:Full timeShift:Day ShiftDescription:Come join our newest program supporting the ACO Clinically Integrated Network, affiliated with our Medical Group Practices at Trinity Health Of New England!Position PurposeThe Community Health Worker (CHW) supports the care management and coordination activities of the care team in the CIN.What you will doMail educational materials and letters, schedule patient appointments, locate and obtain outside patient recordsCollaborate with community partners, such as VNA agencies, caregiver programs, and DME providersUse general knowledge of medical conditions, nutrition, and diet to educate patientsAssist in locating and updating patient clinical recordsFacilitate patient and caregiver access to community resources programs, Meals on Wheels, senior centers, chore services, etcScreen patients to assess for depression, substance use, and mental health disorders; respond with appropriate interventions and/or referralsMinimum QualificationsHigh school diplomaMedical Assistant Certificate preferred. Associates degree or equivalent medical experience preferredQualified as a Certified Application Counselor for MassHealth program or is willing/able to complete required trainings to support patients in demonstrating eligibility requirementsMinimum of five years' work experience.
