Qualifications:
* High school diploma required; Bachelor's degree in health education, social work, or a related field preferred
* Minimum of 2 years of experience as a Community Health Worker or in a similar role
* Certified Community Health Worker (CHW) preferred
* Proficient in Microsoft Office and able to learn and utilize electronic data systems
* Valid driver's license and reliable transportation

Key Responsibilities:
* Conduct outreach to engage individuals and families in need of health services
* Build and maintain relationships with community members, healthcare providers, and partner organizations
* Participate in community events and health fairs to promote health education and services
* Provide education on health topics such as disease prevention, nutrition, and mental health
* Advocate for clients within the healthcare system and connect them to necessary resources
* Collect and document data on services provided and community health needs
* Collaborate with healthcare providers and social workers to coordinate care for clients
* Attend team meetings, training sessions, and professional development opportunities
Skills & Attributes:
* Strong communication and relationship-building skills, with the ability to work with diverse populations
* Knowledge of local healthcare and social service systems
