Each team, a Registered Nurse paired with Community Health Workers, delivers comprehensive care directly in homes and neighborhood hubs, such as schools, libraries, senior centers, apartment buildings, faith-based organizations, and other identified pop-up locations. The NN team is responsible for collaborating with various stakeholders, including local social services and healthcare providers, mental health and substance use treatment facilities, legal agencies and professionals, housing department, and faith-based leaders to identify and link members of the community looking to achieve wellness in their neighborhoods to NN locations. Health Care Access Maryland (HCAM) will serve as an operational partner for the Neighborhood Nursing Program in urban, rural, and suburban areas over the next three years. ESSENTIAL DUTIESThe primary responsibilities of this position are:Meet with individuals and families in a variety of community settings including homes, community centers, libraries and other hub sites. Collaborate, communicate and coordinate follow up care with Neighborhood Nursing team. Conduct social determinant of health screenings and health risk assessments Link and enroll participants into community resourcesEffectively use motivational interviewing techniques to engage and coordinate care/services to clients.Provide client centered services in a trauma informed approachDevelop and provide health education and training materials for clients and community members. Initiate and assist in developing strong community relationships, attend community meetings and other activities that cultivate trust. Timely data input and documentation in electronic health records and other data management systems. Participation in local/state meetings, conferences, community meetings QUALIFICATIONS:Education, Experience, Certifications and Licensures:Minimum high school education or GED required - Bachelor's degree preferred in Human/Social Services1 year experience working in a Community Health Worker (CHW) role or in another position with the provision of social services, supportive services, personal care, or related dutiesValid Maryland driver's license and insurance requiredMaryland Community Health Worker Certification within 24 months of hireKnowledge, Skills & Abilities:Strong computer literacy skills, specifically with MS Word, Excel, Outlook and SharePoint (365).Sensitivity to cultural and socioeconomic characteristics of population served.
