This includes Freeman West, East and NeoshoOur team collaborates with physicians, nurses, ancillary staff, patients, and their families to provide the best care possibleSchedule: Full Time (40hrs/week)What You'll DoEstablish positive and professional rapport with patients, their families and caregivers and provide assistance in accessing identified resources.Research available resources and establish relationships with community providers to better serve patients and their families.Assist the clinic or community team with development of care plan.
