Learn more about our work at healthsolutions.org.Summary of the Position:Community Health Workers (CHWs), who work within the community and in health care settings, empower patients and their caregivers to improve health outcomes by providing culturally sensitive education, peer-based support, and connections to primary care and community-based social supports. The Pediatric Community Health Worker will receive referrals from medical providers to support children with special health care needs or unstable medical or psychosocial status to connect to clinical and community services to support their health and well-being and to reduce stress. Participating families receive comprehensive information from their CHW regarding: 1) understanding their child's conditions, 2) knowing how to access health care for their child, and 3) learning about self-management to keep their child's condition under control. CHWs also help families to connect to services for issues such as food insecurity, immigration, housing, education, and employment. This role will work specifically with pregnant participants in the NYP Queens health system.Specifically, the Community Health Worker will:* Manages designated caseload and meets all stated program goals.* Conducts home visits to provide in-person support and referrals to services.* Offers practice-based support and information sessions at assigned patient-centered medical home (NY Presbyterian Queens), Public Health Solutions.* Collaborates with healthcare team throughout the patient-centered medical home and hospital.* Completes appropriate documentation of assessments and educational sessions at program intervals.* Maintains organized filing system and submits all paperwork and documentation to program staff by designated deadline.* Facilitates/hosts at least 2 community-based workshops per year.* Conducts community-wide outreach and assists with event planning, including workshops and health fairs.* Attends all required staff meetings and trainings.Qualifications and Requirements:* Bilingual (English/Spanish).* 2+ years CHW experience or comparable experience.* Salary: $48,000.00-$48,000.00Benefits:* Hybrid Work Schedule.* Generous Paid Time Off and Holidays.* An attractive and comprehensive benefits package including Medical, Dental and Vision.* Flexible Spending Accounts and Commuter Benefits.* Company Paid Life Insurance and Disability Coverage.* 403 (b) + employer matching and discretionary company contributions.* College Savings Plan.* Ongoing trainings and continuous opportunities for professional growth and development.At PHS, we place immense value on diversity within our teams, understanding that varied backgrounds and experiences significantly enhance our community and propel us toward our goals.
