The primary purpose of the Community Health Worker (CHW) is to improve the overall health and well-being of University Hospital patients. Develops trusting relationships to create linkages to community resources, physical and mental health services and address social barriers to care that impede improvement in one's health. The CHW is an integral member of the multi- disciplinary team that works to endorse clinical teams to the community. Provides perspectives to enhance care management strategies while efficiently navigating community resources to provide patients with access and education on the importance of preventive care screenings and services.
High School Diploma or equivalent required. Associate's Degree preferred. Two (2) years of recent experience, such as any combination of student, volunteer, or work experience, which involves the greater Newark area (or a similar community), and its community resources is required. Participation in community organizations, and first-hand understanding of issues related to interpersonal violence, health disparities and/or issues central to inner-city communities preferred. Completion of a Community Health Worker training program a plus. Sensitivity to a substance abuse environment preferred. Must be a reliable, active listener that exhibits strong interpersonal skills, and enjoys working with the public, one who can adapt to the ever-changing needs of the community. Must have a basic understanding of use of computers and software applications, and the ability to perform basic internet searches. Must have demonstrated interest in serving the Newark community or a similar inner-city community. Must be able to work independently and seek guidance when necessary as well as work in a team environment. Must have the ability to work with individuals of a diverse background. Bilingual in Spanish, Portuguese, or Haitian Creole preferred.
