The Community Health Worker Supervisor is responsible for supervising the activities of Community Health Workers (CHWs) and providing them with coaching and support to ensure the quality of their work and accurate monthly reports. Specific duties and responsibilities include:
- Provide overall supervision of community-based activities
- Ensure a high quality and timely implementation of community-based activities by the CHWs
- Build and maintain strong cooperation with CHWs to keep them informed about community health activities
- Build strong partnerships with local health facilities and ensure necessary training takes place in the catchment area
- Ensure safe and accurate use of all assistive medical devices i.e., blood pressure monitors
- Create and coordinate the work schedules for all CHWs
- Establish a monthly work plan and calendar of activities to share with CHWs
- Conduct weekly on-site supervision meetings to help support CHWs
- Meet with CHWs weekly to review program success and challenges
- Collect and input activity report data into the funder portal/information system
- Ensure the quality and accuracy of all reports
- Review all presentations on a variety of health-related subjects before community events
- Remain updated and informed on all health-related material that will be shared during presentations
- Update all materials with new information provided by the CDC or NYDOHMH concerning health alerts
- Maintain accurate daily logs, case notes, and files for all participants
- Attend scheduled case review meetings, staff meetings, and assigned/required training
- Other relevant duties as assigned
COMPETENCIES
- Demonstrated supervisory experience required
- Highly Proficient in English/Spanish; ability to read, and write in English and Spanish required
- Ability to troubleshoot and problem-solve proactively
- Experience in community engagement, campaign work, or other forms of direct outreach preferred
- Experience working in underserved communities with a genuine interest in improving community health preferred
- Excellent organizational skills and attention to detail
- Strong communication skills with the ability to speak comfortably and courteously to a diverse population, while maintaining a professional and warm demeanor
- Excellent interpersonal skills and customer service oriented; ability to maintain positive relationships
- Possess an entrepreneurial spirit and a positive attitude
- Ability to ability to acceptand followdirections; verbal and written instructions
- Familiarity with Bushwick, Brooklyn, NY area preferred
- Ability to travel to multiple locations in the community with necessary work supplies and materials
- Ability to lift or carry up to 20lbs
- Valid NYS license preferred
- Ability to be adaptable and flexible with assigned work schedule; evenings and weekends as required
- Ability to exercise good judgment and maintain confidentiality, particularly with client information
- Proficient in Microsoft Office suite, intermediate knowledge in Excel and Word

- Ability to understand the concepts of institutional and structural racism and bias and their impact on underserved and underrepresented communities
- Demonstrated commitment to supporting communities that have experienced systemic oppression and bias (i.e., people of color, LGBTQ+ people, immigrants, justice-involved persons, etc.)
EDUCATION AND EXPERIENCE
- High School Diploma or equivalent (HSE/GED) required
- Community Health Worker Certification preferred
- A minimum of two (2) years of supervisory experience preferred
