Job DescriptionEngage members either face to face in a facility or community setting or over the phoneConduct comprehensive member assessment that includes bio-psychosocial, functional, and behavioralhealth needsUtilize motivational interviewing techniques to help members identify and understand their intrinsicgoals and motivate members to engage in positive behavior changeUtilize active listening to collect and retain member information and incorporating responses as they arepresented to complete assessmentEngage members to participate in the assessment process and collaboratively develop plan of care basedon their individual needs, preferences, and objectivesIdentify urgent member situations and escalate to the next level when necessaryWork with members to develop healthcare goals and identify potential barriers to achieving healthcaregoalsEducate members on health and wellness information related to assist with self-managementCollaborate with members to create solutions to overcome barriers to achieving healthcare goalsIdentify relevant community resources available based on member needsPresent complex case information to interdisciplinary care team and obtain input to member plans asnecessaryUse desk top computer applications (e.g., Excel, Word, Outlook, Office Communicator) to summarizeinformation (e.g., reports) or communicate member information to othersDocument information (e.g., assessment information, member interactions, referrals, follow up plans) inrelevant computer systemsLearn updates and changes to computer systems/applications and apply to navigate applicationseffectivelyExecute and meet performance goals within a virtual environmentRequirementsStrong desire and passion to help and work with members with complex medical, behavioral, or social needsBachelor's degree in social work, licensed practical nurse (LPN), CNA/HHA preferred or HS graduate with3 years of experience working within the community health setting in a health care role or experience withmotivational interviewingMust be fully vaccinated against Covid 19Ability to prove technical knowledge of Microsoft products including Word and Outlook; ability to learnclinical applications quicklyMust have a designated workspace and access to secure high-speed internet via cable/DSL in homeCurrent driver's license, auto insurance, and ability to travel on occasional field visits tomembers/hospitalsMust live in the Philadelphia, PA regionPreferred Qualifications:BilingualMotivational Interviewing
