ABOUT OUR JOBThe Community Health Worker acts as a provider extender by assisting clients with executing treatment plans written by medical providers. Serving as a vital link between health services and community resources, Community Health Workers facilitate access to care, improve service delivery, and provide culturally appropriate health information. The focus is on addressing social determinants of health and working to eliminate disparities in healthcare access.Essential Duties and Responsibilities:* Work collaboratively with patients and community healthcare teams to implement care plans and encourage client compliance with follow-up care plans as directed by medical providers.* Collaborates with Addiction Medicine and Community Health service providers in daily hospital rounds to meet and evaluate potential Community Health Worker clients.* Provide culturally appropriate health information and support for healing.* Work with patients participating in a respite care program.* Establish relationships and maintain strong communication skills to engage patients in their care, articulate clearly and effectively when advocating for patients and community groups, and assist members in understanding and effectively navigating complex health systems.* Maintain a client-centered approach to case management by facilitating access to medical and community resources.* Reduce socio-economic barriers to follow-up healthcare by assisting clients in navigating the healthcare system according to their individual care plans.* Provide critical thinking guidance to empower clients in discovering solutions to their problems, ensuring successful access to follow-up medical care.* Ensure accurate documentation of client benefits by maintaining patient care records and actively tracking billing information.* Access and document expenses for the purchase of incidental supplies necessary for successfully following care plan instructions through contingency management funding available for each client.* Facilitate telehealth visits between clients and medical providers.* Accompany patients to physician and care plan provider appointments* Transport ambulatory patients when necessary.* Provide home visits when appropriate.* Demonstrates strong organization skills, with the ability to anticipate future planning needs is required.* Adherence to all patient privacy laws in health care settings and the community.* Other duties as may be assigned.ABOUT YOUWe understand that your time is valuable and encourage you to apply if you meet the following minimum qualificationsQualifications:* High School Diploma or General Education Degree (GED).* 1+ years in Healthcare, Patient Care, or Home Health (preferred).* A combination of education and experience deemed appropriate by the hiring manager.Certificates, Licenses, Registrations:* Must possess a valid driver's license or the ability to obtain one within 30 days from the date of hire.* Basic Life Support (BLS) Certification within 60 days of hire.* Community Health Worker (CHW) certification within 8 months of hire
