OUTREACH WORKER

General Summary: The Access Services Department initiates all new client enrollments for Thrive. The Access Outreach Worker will be the point of contact for new referrals from BHDDH, DOC, hospitals and other referring entities. The role of this outreach staff member will be to interact with the referring entity as well as the individual being referred to allow a smooth transition into services and a secure bridge from the referring provider.

Essential Responsibilities:
- Engage with Thrive's target population in the community or residential settings to help breakdown access barriers to treatment and create a smooth transition to Thrive.
- Build relationships with state and local referring agencies and respond when needed.
- When notified by a referring agency of a hard-to-reach client, be available in the community to assist with bringing into treatment.
- Work with other colleagues at Thrive to provide needed outreach and services for referred clients.
- Monitor people who do not show up for initial appointments and reach out to provide assistance as needed.
- This position is both office and community based.
- Perform general administrative duties as assigned.
- Carry and enliven a positive Thrive image consistent with the Thrive's Guiding Principles and professional code of ethics.
Qualifications
Education, Experience and Competencies:
- A minimum of a Bachelor's degree in social/health sciences, social work, sociology, psychology, public administration, or related discipline.
- A minimum of 2 years' experience conducting community outreach and coordination.
- Demonstrated knowledge and experience working in the community sector with diverse organizations and with individuals living with a range of diverse needs including disabilities, mental health, cultural diversity, and complex care needs.
- Demonstrated ability to consult and liaise with health and community organizations to coordinate activities.
- Training and utilization of motivational interviewing, preferred
- Demonstrated high level of verbal, written, and interpersonal communication skills.
- Working knowledge and proficiency in Microsoft Office applications.
Certifications, Licenses Requirements:
- Valid driver's license and automobile insurance which meets Rhode Island minimum standards required, or the ability to obtain necessary transportation in order to perform the responsibilities/tasks of the job.
