Resulting in improved coordination of high-quality care, enhanced services for all populations, removal of barriers to care, more efficient delivery of care, improved outcomes, enhanced relationships with the community, and increased patient satisfaction.DUTIES & RESPONSIBILITIES:- Identifies High-Risk Patients- Performs patient outreach to address patient needs- Performs intake/assessment and follow-up as required- Call patients after d/c to verify safe transitions - Ensure patient follow-up appointment with PCP (Lincoln and outside providers) as appropriate- Supports patients in need of assistance with one-on-one contact- Ensures that patients with high-risk diagnoses receive the appropriate care based on best practices- Help the patients navigate through transitions in the healthcare system- Works within the organization and through external services to eliminate barriers to health care - Reaches out to patients to determine reasons for not keeping treatment appointments and/or arrangements as outlined in their discharge plan- Collaborates with community agencies to provide new appointments and/or linkages for those patients who were non-compliant - Refer to Care Manager/RN Visit for patient education, as appropriate- Refer to Social Worker when appropriate - Responsible for Registry Management- Collects data and submits report on activities/interventions and outcomes as identified- Partner with Primary Care Physicians to coordinate and facilitate follow-up- Facilitate Care Conferences for complete patients- Identifies with unit staff problem issues with community agencies and/or gaps in resources- Reaches out to community agencies as representatives for the unit to explore new resources, problem-solve identified issues, and/or develop linkages to facilitate patient referrals - Performs outreach home visits as necessary- Knowledge of and ability to utilize Psyckes database.- Preventing and Managing Crisis Situations (PCMS)- Behavioral Emergency Support Team (B.E.S.T.) intervention training- Incident reporting- Manual restraints review- Face Shield- Performs other duties assigned by supervisor as neededMinimum QualificationsFor Appointment to Level I 1. One year of full-time experience in a government agency or community organization in providing community services to the public or in assisting members of the community in obtaining community services; or 2. Completion of an approved six month training program of combined classroom training and on-the-job experience in community liaison work, plus six months of full-time experience as described in 1 above. For Appointment to Level II In addition to meeting the qualification requirements of Assignment Level I, appointment to Level II requires: 1. Two additional years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or 2. Education and/or experience equivalent to 1 above. Study at an accredited college in sociology, psychology or other behavioral science may be substituted on the basis of 30 semester credits for each year of the experience described above. However, all persons must have at least one year of the full-time experience described above. For Appointment to Level III In addition to meeting the qualification requirements of Assignment Level II, appointment to Level III requires: 1. One additional year of full-time experience as described in 1 above. For Appointment to Level IV In addition to meeting the qualification requirements for Assignment Level III, appointment to Level IV requires: 1.
