Job DescriptionPURPOSE OF POSITION:Under supervision, the Community Liaison Worker's responsibilities will include but be not limited to achieving and maintaining the goals and objectives of the NYCH+H Metropolitan Referral Office. Assist and coordinate healthcare referrals utilizing the EPIC H20 application to procure specialty services for patients at external healthcare facilities. Facilitate and expedite appointments for patients from community-based health care practices or other H+H facilities in our facility. RESPONSIBILITIES:- Review and retrieve on-line or faxed community physician referral forms for processing in EPIC.- Review referrals for completeness with accompanying documentation prior to scheduling appointments.- Assure that all pertinent referral information is entered in EPIC timely and accurately.- Process schedule appointments in EPIC application system.- Mail detailed correspondence to patient confirming appointment, location and list of documents that need to be available on the day of their scheduled appointment and call patient to inform them of the date and time of their scheduled appointment three (3) days prior to the appointment date.- Retrieve consult and test reports and/or results to post on EPIC.- Ensure that Specialty consults and Radiology test results are returned timely to the referring physician via HHC Advantage until the facility accepts EPIC Care Links and their choice application.- Inform referring Physician of patient no-shows via HHC Advantage until the facility accepts EPIC Care links as their choice application.- Register and schedule appointments with PCP for patients that walk-in with a referral from a CBO.- Provide service to patients and their families to facilitate their connection with primary care physicians or ER physicians.- Outreach community providers- Maintain the efficiency and integrity of the referral process while maintaining confidentiality and dignity of patient records, information and activities involving all patient information.- Other related duties as assigned by supervisor.- Retrieve, review and process on-line or faxed referrals for requested service by transcribing into EPIC H20.- Monitor flow of external/internal referrals to ensure they are being processed timely and to ensure there are no back-logged electronic referrals.- Assist in informing referring physicians of patient no-show via HHC Advantage for those non-EPIC facilities.- Maintain and ensure the efficiency and integrity of the referral process in addition to maintaining HIPAA compliance at all times.
Minimum Qualifications1. One year of full-time experience in a government agency or community organization in providing community services to the public or in assisting members of the community in obtaining services; or, 2. Completion of an approved six-month training program of combined classroom training and on-the-job experience in community liaison work, plus six months of full-time experience as described in (1).NYC Health and Hospitals offers a competitive benefits package that includes:
