Job Description:
We are seeking applicants for Community Living Support Professional to assist individuals with developmental or intellectual disabilities in their daily living activities within their community. The primary role is to provide support and guidance to help these individuals lead fulfilling and independent lives. The role may involve assisting with personal hygiene, meal preparation, medication management, transportation, household chores, and accessing community resources and services. Community Living Support Professionals often work closely with clients, their families, and other healthcare professionals to create personalized care plans and ensure the well-being and inclusion of their clients within the community. Community Living Support Professionals promote independence, autonomy, and quality of life for individuals with disabilities.Qualifications/Requirements:Must be 18 years of ageMust have a high school diploma or its equivalentMust have at least one year of experience working with adults with developmental and/or intellectual disabilitiesMust complete all required training as outlined by DDID regulations, including first aid and CPR certificationMust be able to read, understand and implement written and oral instructionsMust pass a criminal background checkMust pass pre-employment and random drug testingMust have a valid driver's license, good driving record, and proof of auto insuranceMust be available to work weekends and holidaysResponsibilities:Assesses skills and interests of clientsProvides direct care and support to individuals with developmental and intellectual disabilitiesDocuments services provided to clients in EVV/OASISAssists individuals with daily living activities, including personal hygiene, meal preparation, medication administration, and transportationFosters a safe and inclusive environment for individuals to promote independence, socialization, and community integrationImplements individualized care plans and documents individual's progressCollaborates with families, caregivers, and interdisciplinary team to ensure the highest quality of carePromotes and facilitates skill development and personal growth for participantsAccompanies participants to community activities, if needed, and facilitates their involvementCooperates and works as a team member with co-workers and supervisors, treating them with respect and promoting teamworkCompletes initial and annual training (CPR, First Aid, Tb skin assessment, etc.) as directed by Agency policy and regulatory standards. Attends monthly staff meetings and additional staff development trainings as scheduledAdheres to Agency policy and procedures. Maintains a consistent work attendance record.
