JOB SUMMARY: As a Community Nurse RN in our Assisted Living facility, you willplay a critical role in ensuring the delivery of high-quality care toresidents. The Community Nurse RN will promote a safe and healthy environmentfor residents while respecting their dignity and independence. RESPONSIBILITIES:
- WoundMonitoring & Documentation
- Conductroutine assessments and document significant changes of condition
- WeightMonitoring
- Medicationorder checks
- GiveIM injections
- Monitorresidents' health status
- Adhereto state and federal regulations regarding healthcare delivery in an Assistedliving environment
- Educatestaff on policies, procedures, and compliance guidelines related to residentcare
- Assistin creating a positive, safe, and supportive environment for both residents andstaff
- Performsother duties as assigned
REQUIREMENTS:- CurrentRN license in the state of practice.
- CPR Certification
- Minimumof 1-2 years of nursing experience, preferably in Assisted living, geriatrics,or community health
- Strongassessment and clinical skills
- Excellentcommunication and interpersonal skills
- Commitment to providing compassionate and quality care to our residents

Address: 365 SW Belair Dr., Clatskanie, OR 97016 WHO WE ARE: One of our newest communities, The Amber was acquired in November 2022. The community has a license for up to 40 assisted living residents and has 32 total units. Located in the tiny town of Clatskanie many of the residents are long time community members. The community has a newly renovated employee breakroom and family like staff. Sapphire provides on going training, tuition reimbursement, competitive wages and generous incentives to all employees.
