Also responsible for ensuring a seamless transition from the hospital to the SNF, acting as a case manager, admissions coordinator and champion through the resident's rehabilitation.Key Qualifications
- Case Manager experience
- 3+ years' experience as Admission Coordinator acceptable.
- Public UAI certified or able to become UAI certified within 30 days.
- Must be highly motivated, energetic and able to work independently.
- Previous experience in care management and post-acute care
- Must possess time management and organizational skills, strong interpersonal relationship skills, and must be supportive of organizational philosophy/policies.
- Strong verbal and written communication skills to provide excellent customer service and have a proven history of census goals achievement are required.

Job Responsibilities
- Organizes, develops and updates an ongoing marketing/admissions plan and presents The Chesapeake SNF, AL and MC to hospitals, physicians, discharge planners, etc.
- Educates hospital patients and their families about The Chesapeake and helps them to select The Chesapeake as the SNF for their rehabilitative stay.
- Completes insurance verification's for admissions and for outpatient rehab visits.
- Collaborates with Maintenance and housekeeping team to ensure room readiness.
- Participates in networking groups. Cultivates referral relationships with medical providers for skilled referrals.
- Assists in hosting educational programs, open houses, speaking engagements and other events to promote The Chesapeake SNF, AL and MC.
- Responsible for working with the Marketing Department on advertising and promotional material on an ongoing basis to keep information about The Chesapeake's SNF relevant and current.
- Acts as liaison between the hospital systems and The Chesapeake's clinical team to ensure successful short-term rehab stays.
- Develop open lines of communications through referral sources regarding clinical appropriateness and recommendations regarding admissions.
- Reports back to the referral sources upon completion of a rehabilitative stay with success stories and thanks them for the referral.
- Provides effective bed management to maintain budgeted census goals
- Gathers and communicates information on hospital referrals. Determines appropriateness of admissions. Confers with DON on questionable referrals prior to denials.
- Completes admission paperwork including: insurance verification's and authorizations, review of clinical documentation, Admission Agreement (with family/resident within 24 hours of admission) and other documents for the SNF.
- Keeps informed of patient progress through ongoing communication with The Chesapeake clinical team including the nursing staff and therapists.
- Acts as a case manager, starting the discharge planning upon admission in collaboration with the social worker and IDT team.
- Collaborates with The Chesapeake Social Workers as needed to ensure appropriate discharge planning is carried out to include outside referrals to home health, DME companies, etc.
- Completes Medicare Required Notices-ABNs for Part B Medicare Rehab and receives/processes any notices for Part A audits (ADRs).
- Accepts assignments during community disaster responses, inspections and drills.
