Outreach is seeking a qualified candidate for the position FT Community Support Specialist for our adolescent residential program located in Brentwood, LI. The Community Outreach Specialist is responsible for coordinating care and participating in discharge planning. Duties include addressing case management needs of adolescent clients and their families including access to employment, education, entitlements, medical and psychiatric services, housing, socialization, and other supports. Scheduled hours are Monday and Friday: 7 am to 3 pm; Tuesday, Wednesday, Thursday 8 am to 4 pm.
