Job Description:

The LA County Liaison will play a pivotal role in building and nurturing relationships with Community-Based Organizations (CBOs) in the Los Angeles area, specifically those focused on supporting transitional-aged youth (TAY), former foster care, and other system-involved youth. This position will work closely with CBOs to reduce administrative burdens, increase participation in Medi-Cal programs, and support the success of smaller, community-led Transitional Housing Program (THP) organizations. They will also be the Full Circle representative, working with the public departments, Mental Health, public health, probation and other social service agencies as needed.

Key Responsibilities:

Relationship Building:

  • Develop and maintain strong partnerships with LA-based CBOs, particularly those led by individuals from the communities served.
  • Engage with CBO leaders to understand their needs, challenges, and goals, providing tailored support to enhance their capacity and sustainability.

Program Support:

  • Assist CBOs in navigating the complexities of Medi-Cal participation, ensuring they meet program requirements and maximize available resources.
  • Coordinate training and technical assistance for CBOs on ECM, CHW Services, and Community Supports.
  • Collaborate with FCHN's internal teams to align support services with the needs of LA-based CBOs.

Advocacy and Outreach:

  • Serve as a liaison between LA-based CBOs and FCHN, advocating for the needs and perspectives of smaller organizations within larger policy and program discussions.
  • Conduct outreach to identify and engage new CBO partners, particularly those serving TAY populations.
  • Connect FCHN providers to county partners, hospitals and other organizations to advance the reach and implementation of CalAIM.
  • Develop CalAIM toolkit for key community stakeholders including the Child Welfare Department, Specialty Mental Health providers and Housing providers to support the whole-person, integrated care model

Provider Development:

  • Regularly meet with local CalAIM teams, providers and other key stakeholders
  • Attend outreach events in Los Angeles County as needed
  • Provide education on FCHN and CalAIM services to new providers serving children and youth in Los Angeles

Grant Management and Reporting:

  • Support the Catalyst Center and FCHN in managing grant-funded initiatives aimed at enhancing the success of small, community-led THP organizations.
  • Track and report on the progress of partnership initiatives, ensuring alignment with grant objectives and FCHN's mission.

Education:

  • Bachelor's Degree in Social Work, Public Health, Community Development, or a related

field. Master's degree preferred.

Experience:

  • Minimum of 3 years of experience working with CBOs, particularly in a liaison, program coordination, or partnership development role.
  • Demonstrated experience supporting organizations focused on transitional-aged youth, foster care, or system-involved populations.
  • Familiarity with Medi-Cal programs, ECM, CHW Services, and Community Supports.

Skills:

  • Strong interpersonal and communication skills, with the ability to build and maintain effective partnerships.
  • Knowledge of the LA County social services landscape, particularly in the context of homelessness and youth services.
  • Ability to navigate complex administrative and regulatory environments.
  • Strong organizational skills with the ability to manage multiple projects and priorities.

  • Ability to travel within LA County to meet with partners and stakeholders.
