Promotes a positive physical and psychosocial environment for the residents.ESSENTIAL DUTIES AND RESPONSIBILITIESCreates a resident-centered approach to the planning and delivery of services to residents, supporting residents' rights toparticipate in decision-making and treating residents with dignity and respectPerformsassessments on health, functional and psychosocial status of residents.Participates in the development ofindividualized service plans. Proactively manages care and services for eachresident, maximizing the resident's opportunity to remain in the assisted living environment. Problem-solves as partof the management team to case manage and meet the needs of residentsAssesses or evaluates residents with reported changes of condition. Provides direction, service planning,and coordination of care to meet the residents' changing needsCollaborateswith the management team on the move in of prospective residents, taking intoconsideration the needs of the prospective resident, current community acuity, and staffing levels. Provides in-person pre-move inassessments, if necessaryEnsuresthat resident care needs are met by coordinating services with otherprofessionals, including physicians, discharge planners, nurses, home healthagency staff, case managers, hospice, and state agency personnel. Oversees the arrangement of ancillary services such as pharmacy, podiatry, lab, andx-ray as neededDelegatesnursing tasks as allowed under applicable state nursing laws and practice acts,communicating regularly with residents, their families, and their health care practitionersOversees the documentation and communication of resident care and services. Assists the leadership team to ensure resident recordsare completed in a timely manner and maintained according to company policiesand state regulationsMaintains and oversees medication services including staff training, staff oversight, pharmacycoordination, medication room, and medication systemsPartners in the investigation and root cause analysisof incidents. Collaborates with the management team on the development and implementation of appropriate interventionsAssists in the management of Infection Control. Assists with providing training to staff on infection controlpractices, investigates, and reports incidents of infection and tracksinfection dataProvides on-call support and phone triage for team members' questions regarding resident concernsParticipates in leadership team by actively contributing in decision making, incident follow up, building and company-wide initiatives and attending leadership team meetings such as daily stand up, Systems Monitoring and Resident Tracking (SMART), Quality Assurance Performance Improvement (QAPI), and othermeetings as requiredActs as an ambassador of Prestige by representing communities in a positive manner to outside agencies and participating incommunity events and social functionsMaintains up-to-date knowledge of all rules,regulations and policies pertaining to resident care. Responsible to stayinformed of industry and health care trends and standards, applying newknowledge to meet the changing needs of residents. Delegation experience in thestate assigned preferred.
