Position Summary The Community Resource Navigator is a key link between individuals and the resources they need to thrive. Reporting to the Director of Behavioral Health, this role involves assessing client needs, developing service plans, providing case management, and connecting clients to vital healthcare and social services. As a Community Resource Navigator, you will build trust with clients, engage with at-risk populations, and collaborate with partners such as healthcare providers, housing agencies, and social services organizations. You'll monitor client progress, facilitate access to support services, and ensure plans are tailored to individual goals. Qualifications
