Job Description:

Public Partnerships LLC supports individuals with disabilities or chronic illnesses and aging adults, to remain in their homes and communities and self-direct their own long-term home care. Our role as the nation's largest and most experienced Financial Management Service provider is to assist those eligible Medicaid recipients to choose and pay for their own support workers and services within their state-approved personalized budget. ?We are appointed by states and managed healthcare organizations to better serve more of their residents and members requiring long-term care and ensure the efficient use of taxpayer funded services.

**Duties & Responsibilities:**

+ Responsible for successful service delivery, outcomes, and related service level agreements across assigned programs

+ Coordinates and oversees referral and lead generation/referral growth, strategy, and approach across assigned programs.

+ Ensures program operations and service performance metrics by staff and program are met; including adherence and performance improvement opportunities are executed when required.

+ Guides and mentors' staff and supervisors to facilitate team meetings and employee engagement activities, maintaining a strong and cohesive workforce.

+ Constantly informs supervisor and staff knowledge, skills, and abilities, brokering resource development and training to enhance team performance.

+ Provides leadership, mentorship and training to new and existing enrollment supervisors and staff, spearheading professional and workforce development within the team.

+ Identifies opportunities to develop and create new approaches, training materials, and other resources facilitating successful enrollment into self-direction of services, education, and support.

+ Visit a diverse population of participants in various home settings.

+ Spend at least 60% of working hours on home visits, with flexibility for increased visits as needed.

+ Organize home visit caseload efficiently.

+ Drive 60+ miles to participants' homes, maintaining a satisfactory driving record and a reliable, insured vehicle.

+ Support the organization's mission and improve its community image.

+ Represent PPL and state programs at community events to build awareness and support.

+ Provide linkages and referrals to PPL from other community programs.

+ Collaborate with other state program areas to enhance service delivery and relationships.

+ Identify service gaps and bring back resources for staff and clients.

+ Create and implement organizational initiatives.

+ Provides in-home and over-the-phone enrollment support.

+ Assess community needs and utilize available data effectively.

+ Participate in committees and workforce initiatives.

+ Deliver presentations and training to staff, partners, and participants.

+ Organize and coordinate community events.

+ Support job development, employment services, and educate participants on self-direction and program enrollment.

+ Attending regional events to build the organization's brand and educate families and participants on self-direction and enrollment.

+ Learn and master PPL systems.

**Required Skills:**

+ Independently manage a caseload with excellent time management and organizational skills to meet contractual timelines.

+ Strong verbal and written communication skills.

+ Strong interpersonal, engagement, organizational, and time-management skills.

+ Understand, support, and explain the core tenets of self-direction.

+ Function effectively as part of a team, communicating and partnering with respect and professionalism.

+ Maintain timely documentation: complete in-home visit notes during the visit and upload them the same day.

+ Commitment to supporting diverse and underserved groups.

+ Entrepreneurial approach to relationship development with external stakeholders.

+ Empathetic approach to solving participant issues and challenges.

+ Display understanding and sensitivity to cultural diversity.

+ Proficiency with computers and Microsoft Office software.

**Qualifications:**

+ A bachelor's degree in social work, Human Services, or Sociology strongly preferred.

+ Minimum two (2) years of experience providing mentorship, guidance, and training to team members or direct reports.

+ 2+ year of relevant experience working with vulnerable populations in their homes and engagement efforts related to delivering self-determination, job development, and support-group resources to diverse and underserved communities.
