Compensation: $25.54 hr/$26.81 hr (master's degree)Due to critical service need of the communities we serve, we are offering a $1500 sign-on bonus.* Additional $500 sign-on bonus for Qualified Mental Health Professionals. We are eager to continue to provide quality and compassionate care in our service areas and need you to make this happen!The Community Support Specialist CSC provides integrated, behavioral health community support services engaging individuals experiencing First Episode Psychosis (FEP) and their family in learning and practicing skills to meet their goals and increase functioning. Services are provided when needed, during the day, evening and maybe on the weekend.ESSENTIAL FUNCTIONS* Assess and address the needs of each individual, provide direct services, and/or make referrals ensuring individualized needs are addressed* Coordinate all aspects of care for the individual, including but not limited to locating resources like housing, transportation, benefits, support groups, and other community resources* Attend team meetings, coordinate care for the individual, and participate in treatment planning* Complete documentation collaboratively with client and family and complete all required documentation within the required timeframe, according to agency policy* Services are provided in the home, community and school with the client and family based on clinical need, including evenings and weekendsCommunity Support Clinical Service Provision* Using a motivational, stage-matched approach, provide individualized services based upon strengths, needs, abilities and preferences identified collaboratively with client/family focused on supporting client and family's movement toward improved functioning and achieving their treatment goals* Work with family to engage and participate in treatment with client is required and necessary for improved clinical outcomes* Provide services to client/family using System of Care philosophy; and a holistic, person-centered, strengths-based perspective, incorporating coaching surrounding the Eight Dimensions of Wellness, with attention to each domain of wellness, including physical and mental health* Support client/family in completion of goals identified on Wellness Plans, reviewing the WP with client/family as required, reassessing client/family need, service choices available, community functioning and as needed for updating of goals and objectives as items are achieved, or as client/family goals change* Assist the family with accessing, locating and arranging services and resources to assist in achieving their goals* Maintain contact with client/family when client is hospitalized, participating in discharge planning and communicating with all involved providers in developing after care plans* Provide educational information and skill building to client/family regarding areas of concern such as daily living skills, coping skills, education, symptom management, communication/relationships with focus on improving functioning* Develop and implementation of a crisis intervention plan with the family and provide crisis intervention services as needed, during and after normal working hours* Assist client/family in developing natural support systems including work with family members, school staff, legal guardians, etc.* Participation in and coordination with the child and family interagency team members on an ongoing basis to advocate for the Mental Health needs of child/familySupported Employment & Education* Works with individuals, individually or in a group and engages with families, caregivers, and/or social supports when needed/desired (with permission)* Collaborates and works closely with team members to optimize engagement in the recovery process* Assesses individual's current academic functioning skill level, employment history, prior skills training, and any barriers to vocational involvement* Assists with job development, job search services, and interview preparation that are directed towards positions that are individualized to the individuals' interest and needs* Documents activities, services, and referrals provided in accordance with departmental policies and procedures* Assist individuals in attending job fairs, visiting school sites, and other occupational/rehabilitative duties* Engages in outreach to employers and maintenance of contact with employers in the community.* Provides services with a flexible schedule evening and weekend hours* Other duties as assignedIntegrated Health Team* Communicate actively with other members of the integrated team, including psychiatrists, therapists, primary care physicians, specialists, (Family Support Provider, CSTAR, etc) nurses, dentists, NCMs, etc. regarding all areas of client wellness, including client's adherence to medications, side effects and efficacy of medications* Seek and include input from Integrated Health Team providers on client's Wellness Plan* Make referrals to additional services/providers as needed for client's Integrated Health Team* In addition to clinical providers of the team, function as a liaison to others involved in client's treatment including, but not limited to family, school, Children's Division, Juvenile Office, guardians, etc.Miscellaneous* Will seek culturally competent services and service providers to meet the unique cultural needs of each agency client and family* Driving is an essential function of the position* High School/GED required* Associate's/Bachelor's/Master's degree in a human service-related field preferredWORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS* At least four (4) years of experience OR a combination of higher education and experience required if a High School/GED is the highest education obtained* At least two (2) years of experience OR a combination of higher education and experience required if an Associate's degree is the highest education obtained(*If the degree is an Associate's of Applied Science in Behavioral Health Support from an approved institution;
