Job Summary:

The Community Support team member will provide comprehensive support services to clients with mental health issues and their families. The position includes client care in a primary care setting and in the community and will involve direct contact with people requiring empathy and sensitivity. The Community Health Worker will be responsible for establishing and maintaining interpersonal relationships with clients and their families. This individual will function in a multidisciplinary team providing care for clients with addiction, social, and psychological needs and will be provided with intensive and on-going training in these areas.

Roles and responsibilities will include the following but not limited to:

Participate in the intake process by collecting, organizing, and analyzing information about individuals or families.

Provide direct casework services for social involvement, health needs, and community life to individuals or families.

Prepare and collaborate on various reports and develop case presentations on assigned individuals or families.

Perform regular visits to provide patient and family support and education.

Coordinate appropriate programming, resources, and services for social development and in preparation for individuals or families.

Serving as the point of contact in absence of the Manager, when applicable.

Collect data and write brief reports to meet evaluation needs of the program.

Complete IM+CANS Recerts.

Make appropriate referrals to health and social service agencies. Referrals may include application assistance and client advocacy within and external to organization.

Qualifications:

A high school diploma or equivalent

Experience with navigation of local medical and social support systems preferred

A minimum of two years' direct experience

Experience with Microsoft Office applications or other similar software applications is required

A valid driver's license is required

Minimum age requirement: Must be at least 18 years of age
