Open Skys Community Supports Program is a Mass Health funded navigator service that provides community case management and care coordination to individuals in the community. Individuals served in the program must have a mental health disorder or co-occurring disorder and be considered at high risk for admission to a 24-hour treatment setting.
The Community Support Navigator engages with members as they enter the treatment system and supports them in gaining access to treatment services and community resources. They will provide case management and direct outreach support to their assigned caseloads, which consist of a combination of members enrolled in Community Support Program, Recovery Support Navigation and/or Community Support Program for People Experiencing Chronic Homelessness.
