Certified - Position - Other, please specify Job Number 370####### Start Date Open Date 08/20/2024 Closing Date Job Description POSITION TITLE: Community Transition Specialist REPORTS TO: Senior Supervisor OVERTIME STATUS: EXEMPT PAY GRADE: 374 JOB SUMMARY: The Community Specialist is responsible for collaborating with the Special Education SeniorSupervisors who oversee transition programs and services including support to teachers, otherstaff, administrators, students, and Portsmouth Public Schools community programs. To includefocusing on social/emotional behavior support, coordination of social programs or any otherenvironmental factors, cultural, or economic disadvantages that impact student academicprogress. ESSENTIAL DUTIES:The following information is intended to describe the overall nature and scope of the work beingperformed in relation to the position. This is not a comprehensive listing of all responsibilities ortasks; other work may be assigned when deemed appropriate:
  • ? Works collaboratively with the special education personnel in supporting social skills, work ethics and post-secondary activities.? Shares in the planning and delivery of transition presentations for teachers and other staff.? Assists with coordination of community resource programs to support post-secondaryneeds.? Coordinates with the business community to establish and/or expand community worksites and employment opportunities for students.? Assist senior supervisors with resource planning, work-based experiences, job training, and independent living skills.? Works collaboratively with senior supervisors, school personnel, community agencies, and families to assist with career and transition opportunities for students.? Provides short-term individual and group counseling to students with disabilities who areexperiencing social/emotional/behavioral problems that may interfere with theirlearning/work performance.? Assist with mental health concerns, behavioral concerns, positive behavioral support, academic, consultation with teachers, parents, and administrators as well as provideindividual and group counseling.? Developing intervention strategies to increase academic success.? Assisting with conflict resolution and anger management.? Provides information to families and assists them in accessing long-term or intensivecounseling services, or community-based activities, as needed.? Serve as a resource to families in accessing community support and transition services aswell as providing information about transition topics.? Develop and coordinate a formal work program for students with disabilities includingnetworking in the community to build employment placements for students.
? Develop and coordinate job placements in the district, and in the community.? Schedule job placements for students with disabilities and coordinate any neededtransportation to job sites.JOB SPECIFICATIONS/PHYSICAL REQUIREMENTS: Knowledgeable of policies and procedures relating to special education placements; excellentoral and written communication skills; ability to effectively communicate with students, parents, teachers, and administrators. This is light work requiring the exertion of up to 20 pounds offorce occasionally, up to 10 pounds of force frequently, and a negligible amount of forceconstantly to move objects; work requires walking, and repetitive motions; vocal communicationis required for expressing or exchanging ideas by means of the spoken word; hearing is requiredto perceive information at normal spoken word levels; visual acuity is required for preparing andanalyzing written or computer data, determining the accuracy and thoroughness of work, andobserving general surroundings and activities; the worker is subject to inside and outsideenvironmental conditions, and hazards.
