This position serves patients with compassion and respect, providing personal care, palliative care, and hospice services to ensure they receive the care they need with dignity and grace. This role includes caring for patients within a defined service area.
As a Hospice RN/RN Case Manager, you will:
- Report directly to the Administrator, Executive Director, or Patient Care Manager Senior while maintaining primary control and professional management of each patient.
- Act as the main liaison between the physician, patient/family, and hospice team.
- Deliver direct patient care services and supportive care to the patient's family while fostering a trusting and compassionate relationship.
- Empower caregivers by educating them on how to care for their loved ones.
- Collaborate with an interdisciplinary team, including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff, to provide comfort and dignity.
- Assess patient and family needs, assisting in the development and execution of personalized care plans.
- Maintain all assigned patient clinical records diligently.
- Engage in quality assessment and performance improvement initiatives.
About You: Compassionate Hospice Nurse/RN Case Manager Requirements:
- A caring and empathetic approach to patients and their families.
- Ability to provide care in various home environments.
- Flexibility and composure in offering support during challenging times.
- Active RN license in the state of intended employment (or eligibility to obtain it).
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
- Experience in hospital settings such as Med Surg, ER, critical care, or telemetry is highly valued.
- Valid driver's license and automobile insurance are required.
- Current CPR certification is mandatory.
