You Will (Responsibilities)
Demonstrate knowledge of competencies in discipline-specific patient care skills required by the care center to provide client care.
Perform comprehensive client assessments as assigned and collaborate with a multidisciplinary care team to develop or implement an individualized plan of care as per physician orders that ensure quality and achieve desired outcomes and goals.
Maintain continuity of client care by communicating information timely and effectively with all team members providing care to the patient.
Advocate for client needs with physicians and ancillary providers.
Completes documentation timely, accurately, and at the point of care, according to industry standards, conventions, and guidelines.
Educate the client, caregivers, and family on the aftercare or ongoing care of the patient
Perform vital signs and client assessment during visits. Assist with ADLs and custodial care needs.
Provide in-home skilled care to clients including but not limited to:
Wound Care
Medication administration
Trach care
G/J-tube management
Post-operative care
IV Therapy/Injections
End of life care
Ventilator care
You Have (Qualifications)
Current and unencumbered Registered Nurse or Licensed Practical Nurse License in your practicing state or eligible compact state licensure
1+ year(s) of experience as a RN or LPN
Current CPR certification
Technical tools and software/EMR for visit documentation knowledge and experience
A reliable method of transportation, current drivers license, and car insurance coverage
Proof of Immunization/Exemption: Flu, and COVID-19
TB Test or Chest X-Ray
A willingness to complete additional training, as assigned
Demonstrate competency in delivering comprehensive and personalized care to clients, including but including (but not limited to) the following:
Assisting patients with the completion of activities of daily living (ADLs)
Monitoring vital signs - reporting findings to the appropriate nursing personnel
Perform client care as assigned, and collaborate with a multidisciplinary care team to deliver care to clients both at home and in the community.
Maintain continuity of patient care by communicating information timely and effectively with all team members providing care to the patient.
Complete documentation in a timely and accurate fashion, and at the point of care, according to industry standards, conventions and guidelines.
Maintain the comfort, privacy, and dignity of each client in the delivery of services. to them.
Knowledge of UFully understand all aspects of clients' rights, including the right to be free of restraints and free of abuse.
Responsible for promptly reporting to the clinical office or administrative staff incidents or evidence of resident abuse or violation of residents' rights.
Assist in maintaining a safe, neat, and clean environment.
Observe clients for changes in condition or behavior and promptly report these changes to appropriate licensed nursing personnel.
Communicate and interact effectively and tactfully with the client, visitors, families, peers, and supervisors.
Adheres to companyto all company policies, procedures, and standards of conduct.
APerforms additional duties as assigned.
Current and unencumbered license to practice as a Geriatric Nursing Assistant License in your practicing state or eligible compact state licensure
6+ months of clinical experience as a GNA
Current CPR certification
A reliable method of transportation, current drivers license, and car insurance coverage

A working and reliable vehicle with active automobile insurance
A willingness to complete additional training, as assigned
