Job Description:
*Responsible for:*Under the direction of the Quality Assurance Director, the Continuous Quality Improvement Analyst is responsible for ensuring that the work submitted by staff is consistent with SCHARP philosophies and meets the current standards for quality of care, and other requirements as defined by L.A. County Department of Mental Health and SAMSHA. This position will follow documentation and chart review procedures set forth by the LAC-DMH Quality Assurance department and Certified Community Behavioral Health Clinic. Continuous Quality Improvement Analyst will also function as a team member on the accreditation committee and analyze outcome data associated with the SAMSHA-CCBHC grant.*Position Qualifications: ** Master's Degree required, license preferred* Experience with documentation requirements for respective funding sources, specifically Medi-Cal, LAC DMH and SAMSHA funded programs.* Knowledge of Los Angeles County Department of Mental Health's Rehab Option model of service provision and documentation requirements preferred.* Must be able to pass California State Department of Justice criminal background investigation, Live Scan and/or fingerprinting.* Experience with Electronic Health Record (Clinitrak).* Possess attention to detail, ability to be thorough and accurate with excellent data management skills.* Demonstrate ability to handle multiple tasks simultaneously and to work independently and creatively* Excellent communication, interpersonal and team building skills* Must possess a valid California Driver's license and maintain an insurable driving record.* Must be able to pass California State Department of Justice criminal background investigation, Live Scan and/or fingerprinting.* Strong proficiency in Microsoft Office Suite, with advanced expertise in Excel.* Work with minimum supervision* Maintain a safe work environment and confidentiality at all times* Combination of field and office environment* Be proactive, creative, and flexible in determining, evaluating, researching, and resolving issues* Maintain a professional demeanor that reflects positively on the agency* Demonstrate respect and courtesy toward others* Able to thrive in a work environment emphasizing teamwork and collaboration* Respond promptly in all aspects of communication*Primary Duties:*Reporting to the agency's Quality Assurance Director, Continuous Quality Improvement Analyst is responsible for the following:· Assist CQI Director in implementing CCBHC CQI plan by collecting, analyzing and evaluating data in support, including clinical outcomes, EBP fidelity, and CCBHC specific events.· Track consumer suicide deaths or suicide attempts: consumer 30-day hospital readmissions for psychiatric or substance use reasons; and such other events the state or applicable accreditation bodies deem appropriate for examination and remediation as part of a CQI plan.· Assists in Policy and Procedure development.· Complete all documentation audits, data reviews, other documentation in accordance with SCHARP/BAFMA policies and procedures.* Review client information in the Clinitrak database: open client episodes and electronic health records, open/discharge clients in database.* Ensure compliance with LAC Department of Mental Health standards by verifying all documentation accuracy within client charts.* Using the audit tool, conduct regular non-clinical and clinical quality checks on charts as directed and per department procedures, including quality assurance reviews, while ensuring all corrections are done in a timely manner to enhance quality improvements in the mental health program.* Demonstrate expertise in maintaining quality control activities including, tracking document submissions and interpreting statistical information to ensure documentation accuracy.· Collect and analyze outcome data associated with the SAMSHA CCBHC grant by monitoring staff's completion of required Outcome Measures to ensure staff are administering instruments and entering information into Clinitrak and/or other identified data collection system.· Ensure that Outcome Measure data is available for SAE evaluator· Develop and present EBP reports from collected data which will measure client's outcomes and can be used in evaluating effectiveness of selected Evident Based Practices.· Provide centralize support in the areas of Evidenced Based Practice (EBP) training, scoring and report keeping.· Collaborate with CCBHC Project Manager to develop a written protocol manual to ensure service delivery reflects the required standards of care for SAMSHA-CCBHC Expansion Project.· Communicate audit findings, Department of Mental Health compliance requirements, and LAC-DMH policy and procedures changes.* Participate in the development of SCHARP/BAFMA written Policies and Procedures manual.* Participate in the agency's accreditation/re-accreditation process.* Develop and maintain accurate and complete client case files to monitor the integrity upon audit findings.* Support data collection for the agency.* Attend and participate in agency's department trainings and meetings, when required.* Organize and prioritize multiple activities to meet all external and internal deadlines* Collaborate and consult effectively with the Program Directors/Supervisors, Quality Assurance Manager, and other staff/programs to ensure Quality Assurance procedures are followed.* Generate Clinitrak reports as needed to assist staff with compliance.* Other duties not specified in the position description as requested by the Quality Assurance Manager or Chief Executive Director.*Environment/Working Conditions*· Routine duties involving interacting with a variety of individuals within the agency, clients, and the community.· Daily functions which may include exposure to clients/family members from various ethnic, cultural and socio-economic backgrounds, with communicable diseases and some may be prone to violence.· Strives to be flexible and adapt to change· Seeks opportunities to facilitate change and encourages others to do the same· Maintains a constructive and positive outlook· Travels to site locations and outside locations as needed or requested.· Position requires driving, valid driver's license, a motor vehicle clearance, and proof of insurance is required and must be maintained throughout employment.
