Department:Women's Way/ Outpatient Treatment Servies Position:Continuum Counselor Reports to:Clinical DirectorStatus:Full Time, Part time Schedule:Flexible JOB SUMMARY:The primary role of the Continuum Counselor is to provide direct substance abuse treatment counseling services, including treatment planning, individualized support, and facilitation across the continuum of services at FTS and help ensure positive treatment outcomes and transitions between levels of care. QUALIFICATIONS: Master's degree in a behavioral health discipline preferred and/or CSAC required. If no CSAC, then there needs to be a plan for obtaining CSAC within one year.Preference for a minimum of two years' experience in a residential substance use treatment facility and/or supervisory or managerial role in substance use treatment. Personal vehicle and valid Hawaii State driver license required. ESSENTIAL FUNCTIONS:
- Develop clinically appropriate treatment plans with assigned clients and revise as scheduled.
- Provide individual substance abuse counseling, group therapy, family counseling, and case management services as assigned.
- Provide classes and psycho-educational groups related to addiction, relapse prevention, family issues, women's issues and/or other appropriate topics.
- Maintain accurate and up-to-date clinical records for each assigned resident, noting significant problems and progress (using the WITS system).
- Prepare written reports and correspondence related to case management in a professional and timely manner.
- Provide ongoing written or verbal communication to other program staff or referral agencies related to client needs or concerns to assure coordination of services.
- Participate in treatment teams across the continuum of services at FTS in accordance with assigned clients.
- Work closely with Clinical Supervisor to support positive structures, procedures, policies, and training across the treatment continuum and for the treatment teams.
- Keeps up to date on agency training, and other pertinent professional development training and requirements.
- Maintain work area in a neat, orderly and sanitary manner.
- Correctly and promptly follow specified agency procedures required for the appropriate handling of all Emergency Response Codes.
- Maintain all credentials and certifications, as specified by agency policy and procedures.
OTHER FUNCTIONS:
- Performs other related management duties as assigned.
OTHER REQUIREMENTS: Able to meet basic standards of physical, mental health, and communication demands.Specified by State and/or professional accreditation standards applicable to this agency.Initial/Annual criminal background check, pre-employment, annual physical examination, CPR/first aid, TB test, and driver abstract compliance with agency policy are required.
