Job Description:

Case Managers helps individuals with disabilities get the support they need to live a fulfilling life. They work closely with each person to create personalized plans, connect them to Medicaid services, and ensure they have the tools to make informed decisions about their lives.

Case Managers regularly checks in, meeting at least twice a year, to make sure the services are still meeting the person's needs. They also handle important paperwork to track services, progress, and any changes in needs, keeping everything accurate and up to date. The role involves being an advocate, a planner, and a connector, always focused on helping individuals achieve their goals.

Responsibilities

  • Develop person-centered support plans in collaboration with individuals, parents, or their legal guardians.
  • Find providers who are able to provide the services the individual needs based on their support plan.
  • Authorize services and develop a budget and submit to governmental agency for approval.
  • Connect individuals with providers and services that align with the individual's unique needs.
  • Provide updates to the individual, providers, parents, or legal guardians to determine and address the individual's evolving needs.
  • Meet the individual in their own home or a safe location at a minimum once every 6 months. Though some situations will require more meetings.
  • Ensure services provided by providers comply with individual wants and needs, while adhering to any local, state, or federal policies.
  • Adjust support plans as needed due to internal or external factors influencing the individual's wants and needs.
  • Advocate for the individual's needs, wants, and independence, including the right to take risks.
  • Document all relevant work activities for the day in the case noting system.
  • Developed support plans using governmental online platform.
  • Conduct audits on your client's case files to ensure compliance with local, state and federal policies.
  • Stay up to date to any service changes proposed by local, state or federal agencies.

Qualifications

  • Accredited four-year degree in social work, special education, psychology, nursing, human services, or related fields.
  • Or an accredited four-year degree with a major in any field and one year of experience as a social worker/case manager/care coordinator in a public or private social service agency.
  • Effective written and verbal communication skills.
  • Strong organizational and time management skills.
  • Intermediate computer and typing skills for accurate case note documentation.
  • Problem-solving skills and ability to multitask.
  • Capacity to work independently and collaboratively within a team.
  • Quick learner, adaptable to a constantly changing environment.
  • Ability to handle challenges with a caring and compassionate approach.
  • Possession of a Driver's License and car insurance, as the role requires visiting clients in their home around the Twin Cities and rarely beyond the metro area.
  • Successful clearance of DHS background check.

Optional Qualifications:

  • Bilingual proficiency.
  • Additional one year of experience in education or treatment of people with developmental disabilities or related conditions, or completion of a relevant course.

Schedule:

  • 8-hour shift.
  • Hybrid: Staff will gain up to 4 days working from based on meeting performance criteria.
