Overview:Carle Health is seeking a full-time Advanced Practice Provider to join our Convenient Care Department in Bloomington, Illinois.Responsibilities: Provides health assessment for patients directed toward continuing a state of wellness, establishing the need for intervention, evaluating responses to and outcomes of therapy, and following a mutually agreed upon treatment plan. Ensures documentation of care is complete and accurate. Communicates with other health professionals, families, and referral agencies regarding health status and needs of particular patients or patient groups. Serves as a role model in the enhancement of nursing practice and professional conduct following professional practice guidelines. Responsible for nursing care delivery by patient care support staff. Conducts or participates in multidisciplinary rounds and ensures appropriate and quality care for assigned patient. Interprets and evaluates findings of studies/tests Recognizes deviations from normal in the physical assessment. Works in collaboration with a physician in formulating treatment plans for health problems and follow-up. Relays appropriate information regarding patient care to the collaborating physician Writes prescriptions for medication, blood products based upon laboratory results, routine diagnostic and follow-up studies, therapeutic measures, and post-discharge care in accordance with written practice protocols Qualifications: License/Certifications:Basic Life Support (BLS) within 30 days - American Heart Association (AHA); Licensed Advanced Practice Nurse - Certified Nurse Practitioner (APN-CNP) - Illinois Department of Financial and Professional Regulation (IDFPR); National APN Certification - Varies; Licensed Registered Professional Nurse (RN) - Additional State - VariesEducation:Master's Degree: Nurse Practitioner ProgramOther Knowledge/Skills:
