I hereby agree to a criminal record identification and fingerprint check pursuant to ORS 181.536-181.537. If the job description requires a valid driver's license, I also grant permission for TFS to make inquiry concerning my driving record for the past three years. I understand that information received as a result of these inquires may affect my employment.
Although management makes every effort to accommodate individual preferences, business need may at times make the following conditions mandatory: overtime, a rotating work schedule, or a work schedule other than Monday through Friday. If selected, I would accept these conditions as part of my employment.
