This is a teleworking position with the expectation that you will be visiting clients regularly within your community region.Who We AreThe Coordinating Center has over 40 years of experience supporting children, youth, adults, and seniors with complex medical needs and disabilities statewide. Over the past four decades, we have become an industry expert in the delivery of person-centered, community-based care coordination services and population health.What You Will Do:The Coordinator of Community Services (CCS) is responsible for targeted case management services for people with intellectual/developmental disabilities. The CCS helps in locating and accessing the full range of medical assistance services, community services (i.e., social, habilitative, vocational, recreational, housing, financial, legal, educational), and other support services. The CCS supports the mission of The Coordinating Center by providing person- and family-centered service coordination within the parameters of the CCS contract.As a CCS You Will:Maintain a gradually developed caseload between 30-40 clients with intellectual/developmental disabilities.Advocate on behalf of clients while supporting self-advocacy and participation in service planning.Assist clients with the development of an appropriate and relevant Person-Centered Plan, in accordance with COMAR regulations and agency policies.Research, access and help implement services and supports in a timely manner, ensuring the most integrated settings to meet the client's needs.Monitor the provision of services to clients by performing regular information gathering and research relevant to the client's supports and services.Visit clients in settings where supports are provided, or other locations as desired by the clients, at minimum four times during the client's plan year depending on the intensity of coordination needed.Assist with all client enrollment-related activities, to ensure continuity of supports and services.Facilitate frequent person-centered planning meetings, including coordinating quarterly visits, annual PCP meetings and interdisciplinary meetings as needed.Conduct required assessments with clients per DDA regulations and program requirements.Maximize clients' participation in the team process and promote opportunities for those who choose to lead their team meetings; negotiate and resolve disagreements within the interdisciplinary team.Annually advise participants of their right to choose from qualified providers of services.Respond to provider-generated incident reports in a timely manner to ensure the individual's health and safety are addressed.Consistently and accurately document sufficient level of client services to meet agency billing standards within State of Maryland's Long-Term Services and Supports (LTSS) system and The Coordinating Center's internal system.Attend and actively participate in all external and internal required meetings and trainings.Maintain a professional relationship with external service providers to ensure quality coordination of comprehensive care.Who You Are:We look for candidates who align with our mission, vision, and values as a human services nonprofit organization. Specific qualifications are:Bachelor's degree from an accredited education program in human services preferred, or specific human service work experience required.Experience in coordinating services for individuals in Medicaid or waiver programs or coordination services for individuals with Intellectual or Developmental disabilities is strongly preferred.Demonstrates competency-based skills and working knowledge in the following areas:Negotiation and conflict management and crisis managementEffective advocacy skills and problem solvingProven ability to establish and maintain working relationshipsPerson- and family-centered careSelf-determination planningCommunity resources and service delivery systems (i.e., generic, local, State, and federal programs, Medicaid, mental health, substance abuse, social services, juvenile justice, vocational rehabilitation, and corrections)COMAR Regulations governing services for individuals with Intellectual/ Developmental disabilities.Strong belief in the opportunities for people with disabilities to fully participate in the community and a willingness to work toward individuals' identified goals.Must have access to a reliable automobile and a valid driver's license.Excellent communication skills-writing and speaking.Computer skills including Word, Excel, and PowerPoint.Ability to manage complicated work processes and multi-task.Why Join UsAs a dynamic teleworking organization, we understand what it means to have a work culture that is dedicated and flexible.
