Job Description:

Under the supervision of the Registered Nurse, Nurse Practitioner, Physician, the Licensed Practical Nurse provides direct patient care for the patient and evaluates outcomes.Demonstrates ability to provide direct patient care, evaluates and adjusts nursing care processes as directed by the Registered Nurse, Nurse Practitioner, or Physician.Communicates clearly and tactfully with patient and/or families. Communicates and ensures understanding of instruction in a manner appropriate for the intended audience.Documents patient teaching and learned information.Assist physicians with examinations, procedures and other processes related to direct patient care as directedExplains procedures to patients thoroughly, provides assistance when necessary, monitors patients comfort and alleviates anxiety.Provides appropriate assistance to physicians during various medical procedures.Monitors the medical condition of patients and reports changes to the Registered Nurse, Nurse Practitioner, or Physician.Demonstrates knowledge of growth and development appropriate to the ages of patients served.Identifies emergency situations and provides immediate care under the supervision of the Registered Nurse, Nurse Practitioner, or Physician.Recognizes changes in patient condition and asks appropriate questions to clarify the situation. Reports changes to Registered Nurse, Nurse Practitioner, or Physician.Communicates pertinent information regarding patient progress and needs to appropriate staff.Ensures accurate maintenance of patient's medical records and that patient records are treated as confidential information both paper records and electronicFollowing departmental standards, completes records promptly, recording all necessary information.Prepares patient for scheduled visit, assists in patient assessment under the direction and supervision of the Registered Nurse, Nurse Practitioner, or Physician.Effectively identifies predictable problems and plans and provides patient care.Completes vital signs, medication reconciliation, patient family and social history, chief complaintReviews patient history on initial visit and updates as neededAdministers medications/injections to assigned patient as ordered by physician/midlevel provider within the scope of LicensureFollows established protocols for medication/injection administration and patient observation.Documents to update patient records.Prepares written paper and electronic documentation as required by the professionReviews medical record to assure all previously ordered test results, lab/x-ray findings are present in record.Make referrals as directed by RN, NP, or PhysicianObtains lab specimens per Physician/NP orders.Functions independently and assumes responsibility for actions in clinical proceduresEnsures compliance with quality improvement policies, procedures and studies.Assists in maintaining a safe, comfortable and therapeutic environment for patient/families in accordance with office standards.Exhibits expertise and skill in understanding, meeting and satisfying the specific needs and behavior of the patient in all age groups.Communicates effectively based on the age of the patients.Demonstrates attitude of cooperation and professionalism.
