Job Description: The substance use counselor is responsible for developing client centered behavioral health services for clients receiving treatment for substance use disorders. As a substance use counselor, you will work with clients to help them develop an individualized treatment plan to help them reach goals and objectives by learning the skills necessary to live free from the burden of addiction. You'll discuss ways to cope and potentially incorporate methods, such as 12-step programs, to help clients toward recovery. Responsibilities: Meeting with clients to evaluate their health and substance problem. Identifying issues and creating treatment goals. Collaborate with counselors, physicians, and nurses to plan or coordinate treatment. Monitor, evaluate, and record client progress with respect to treatment goals. Teaching clients coping mechanisms. Leading group and individual therapy sessions Providing updates and progress reports to care team. Referring clients to community support groups. Setting up aftercare plans. Completing transition of care summaries Completing discharge summaries. Performs other duties as assigned by management. Patient screening and assessment Maintain documentation of clinical supervision Maintain all required CEUs. Qualifications: Must be licensed by the state of KY. Must have a board approved Supervisor (if needed). Master's degree in social work, psychology, counseling, or marriage and family Strong written and verbal communication Prior experience with addiction/recovery and/or behavioral health Excellent interpersonal and engagement skills Strong computer skills and prior EMR knowledge a plus Must pass background check and drug screen.
