- Carries a caseload as assigned by direct supervisor.
- Provides outreach and direct service to consumers in the community in their natural environment to reduce barriers to accessing services.
- Intervenes to change attitudes, beliefs and behaviors of consumers to improve individual and family level of functioning.
- Maintains networking and referral contacts with other human service providers both internally and externally while assessing barriers for successful engagement, treatment, and discharge.
- Completes PATH enrollment documentation and records accurately in the electronic health record system and internal tracking systems to allow immediate supervisor to document appropriately.
- Provides outreach services to engage homeless individuals to link with local resources for shelter, entitlements, and meeting of basic needs at a minimum of 4 hours or more per week.
- Attends mandatory meetings and related PATH conferences or trainings as identified.
- Provides case management services including but not limited to: advocacy, linkages to community resources, housing services.
- Provides crisis intervention to stabilize client's condition by utilizing short term crisis intervention techniques and/or referral to other support services.
