Link to other services and make referrals as needed. Provide regular direct individual contact and provide face-to-face case management services. Meet productivity standards as set for the program. Schedule and facilitate meetings to develop an individualized service plan with involvement of the individual and/or family. Enter necessary information and maintain information in computerized database, state computer systems, and in charts in accordance with state, federal and licensing regulations and Agency policy. Obtain and update information within required timeframes. Provide crisis intervention services as needed. Work in cooperation with and consult with Agency staff, supervisors, other providers, and community resources as needed. Link individuals to transportation services or transport individuals to appointments as needed.Attend meetings, staffings, and trainings as needed. At times, may provide coverage in Somerset County. Perform other related duties as assigned.Work Schedule and Additional Information:* Full-time employment.* Work hours are 8:00 am to 4:00 pm, Monday - Friday, with a 30-minute lunch.* On occasion, staff may need to work hours outside of the set work week to complete projects, meet work demands or in the event of an emergency.* Hours may vary to meet the needs of the consumers.At times, may provide coverage in Somerset County.This position reports to the DBHS Office for work. Upon hire, new staff begin training and are on probation for 6 months. New staff may be eligible for a hybrid of on-site and remote work after 3 months of employment.You will receive further communication regarding this position via email.
