Job Description:

Primary Job Responsibilities Patient Assessment: Assesses and interprets data about the patient#s status in order to identify each patient#s needs and to provide the appropriate care; including physical, social and emotional needs based in age and culture. Uses assessment information to identify and individualize care outcomes to provide direction for the community of care (uses care maps when appropriate).# Identifies and evaluates that patient#s and family#s learning needs, abilities and readiness to learn and documents appropriately. Patient Care: Delivers patient care consistent with the established standards of care, guidelines, care maps, policies and procedures, and regulatory standards.# Formulates and revises the plan of care based on observation and patient#s need that is age specific and culturally sensitive.# Evaluates patient progress toward the attainment of patient outcomes and goals, involving the patient, family and/or member of the healthcare team as appropriate.# Participates in quality improvement activities, developing policies, procedures using evidence based practice and/or guidelines to improve patient care and assists with data collecting and monitoring, Communication: Organizes and prioritizes work assignments based on patient needs and acuity, age and utilizes critical thinking skills. Responds to emergency situations as per protocol.# Maintains complete, accurate and concise documentation of the patient status, nursing interventions, patient#s response to medications, treatments, procedures, and progress toward outcomes. Teamwork: Collaborates as part of and interdisciplinary and interagency team to provide consistent care, communicate patient needs and develop a plan of care.# Participates with interdisciplinary teams and committees to evaluate and improve patient care. Crisis Intervention: Demonstrates advanced knowledge of crisis intervention techniques and ability to coordinate the efforts of the ED and the MHU treatment teams to assess, plan and implement care for the psychiatric patient. Demonstrates the ability to manage the agitated patient, including verbal de-escalation, seclusion and restraint, management of medication and safety needs of the department.# Demonstrates the ability to make effective decisions including in crisis/emergency situations. Safety: Maintains a safe environment for patients. Provides for safe medication administration, minimizes falls and restraint usage. Minimum Qualifications: LCSW, LMSW, or RN. Minimum 1-2 years experience performing crisis evaluations # # # Salary Range: $31.84 - $52.27 Pay Grade: 33 Compensation may vary based upon, but not limited to: overall experience and qualifications, shift, and location.
