UPMC In Home Services is hiring a full-time Staff or Senior Certified Registered Nurse Practitioner to support UPMC Your Care. This is a brand-new program launch. This CRNP has the opportunity to build the program from the ground up. The program provides medical services to patients in their own home. The schedule is Monday-Friday 8:00am-4:30 with on-call rotation.

Purpose:The Certified Registered Nurse Practitioner (CRNP) works independently and in collaboration with physicians to provide care to UPMC Health Plan members living with advanced chronic illness, or potentially life-limiting conditions. The CRNP assists with the diagnosis, treatment, and management of acute and chronic conditions by conducting a comprehensive assessment and development of patient centered plan of care, including assessment and documentation of Hierarchical Condition Category (HCC) diagnoses as appropriate. The CRNP is an integral part of the clinical management team and works collaboratively with the interdisciplinary team to manage the clinical and medical needs of the patients. The CRNP communicates proactively with physicians regarding the clinical condition of their patients. The position will require travel for in-home visits. Must have a valid driver's license in good standing and reliable transportation.The successful candidate will be placed according to years and months of experience for employment purposes.

This position is dependent upon successful attainment and maintenance of hospital privileges, if applicable to the Business Unit practice.

Successful completion of an approved nurse practitioner program required. BSN or MSN preferred. 2 years CRNP experience, plus an additional year experience in specialty area of geriatrics, palliative care, or chronic disease management required. Professional nursing experience as a registered nurse preferred. . Excellent problem-solving skills, self-motivation, enthusiasm, and creativity required. All requirements of the 1st level CRNP must be met. Completion of an OAPP approved compendium detailing professional contributions within one year of hire or promotion into the position. Must have department chair approval for promotion. Demonstration of clinical precepting of one APP student per calendar year for at least two years or evidence of ongoing education of APP students or demonstrate a meaningful contribution to student education. Licensure, Certifications, and Clearances:Cardiopulmonary Resuscitation Cert Registered Nurse Current Prescriptive Authority Approval by the Commonwealth of PA required. Current Drug Enforcement Agency registration (DEA certificate) required. Board Certified in Adult, Gerontology, or Family Medicine preferred. National certification, eligible for licensure by Pennsylvania board of nursing, eligible for certification as a CRNP by Pennsylvania state board of nursing CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.
